Provider Responsibilities

Chorus Community Health Plans (CCHP) offer the support, resources, and education providers need to ensure they are in compliance with our policies as well as the state’s policies. The provider is responsible to follow these policies. It’s the primary care physician’s (PCP) responsibility to ensure coordination and continuity of care with providers, including all behavioral health and long-term care providers. PCPs are also responsible to make sure that our members receive preventive care appropriate for their age group.
