CCHP Pharmacy Services
Effective January 1, 2019, your office may need to submit prior authorization requests for medications differently, depending on if the drug falls to the medical benefit or the pharmacy benefit. Please be sure to read these instructions.
Individual and family plan members get their prescription drugs through our pharmacy benefit manager, Express Scripts. To see if the drug you are prescribing is covered by the member’s plan, you can:
- Call Pharmacy Services for providers, pharmacists and clinical staff at 844-201-4677
- Refer to the Prescription Drug List
- Do an online search
- Complete the Non-Formulary Medications Prior Authorization Form

BadgerCare Plus Pharmacy Services
Our BadgerCare Plus members receive covered drugs, disposable medical supplies (supplied by pharmacies), and certain over-the-counter items from the state through fee-for-service Medicaid. This includes medications obtained at a retail pharmacy and/or self-administered, as well as specialty medication provided in an office or clinic setting.
Wisconsin’s Medicaid pharmacy program includes covered medications and auto authorization restrictions.
