BadgerCare Plus Dental Coverage


BadgerCare Plus fully covers regular dental services for members living in Kenosha, Milwaukee, Ozaukee, Racine, Washington, and Waukesha counties in Wisconsin. For members living in all other counties, dental is covered by the State of Wisconsin, not CCHP. You may see any dentist that accepts your Forward Health card.

  • For members living in Kenosha, Milwaukee, Ozaukee, Racine, Washington, and Waukesha counties:
    • For help finding a dentist, visit our Provider Directory or to make an appointment, call our Dental Customer Service line at 1-877-389-9870. We will help you find a dental appointment within 90 days.
    • If you have a dental emergency and need to see someone right away, call us at 1-877-389-9870. You must attend your emergency appointment.
  • For members living in all other counties:
    • Dental services are covered by the State of Wisconsin, not Chorus Community Health Plans. You may see any Dentist that accepts your Forward Health card. To find a dentist in your area please call Forward Health Member Services at 1-800-362-3002.

If you cannot make it to your appointment, you must call at least 24 hours in advance to reschedule.

  • If you do not call, you will not be able to make another emergency appointment. If you need a ride to your appointment, the state of Wisconsin offers transportation assistance. Call 1-866-907-1493 to request a ride. You must call 48 hours in advance. For emergency appointments, transportation requests may be made less than 48 hours in advance.


Dental and Oral Health Information 

Did you know that tooth decay is the most common chronic disease in children and is also the first cause for school absence? See your dentist at least once a year for a routine cleaning.

The best way to prevent tooth decay is to visit your dentist regularly. BadgerCare Plus and CCHP cover all children and adult’s regular dental services – including teeth cleanings, fluoride, sealants and fillings. Oral health affects physical health and behavioral well-being. It can influence especially how children chew, taste food, grow, speak, look and socialize. While tooth decay and other dental health issues are preventable, many Milwaukee children do not have access to the oral health care they need.
